一、依據教育部114年7月24日臺教學(五)字第1142803363號函及本校「特定人員尿液篩檢及輔導作業要點」第四條第一款規定:每學期開學日起三週內經導師、校內之任課教師、專業輔導人員或職員工觀察後,依特定人員類別提報特定人員名冊,並交由學務處承辦人彙整,並召開會議審查。
二、請各班導師(含系主任)於114-1學期開學日起三週內【114年9月26日(五)下班前】,觀察班上/週遭學生其生理及生活狀態是否符合:「有事實足認為有施用毒品嫌疑之學生。」若有則請登入網路或掃描QR code(如下圖)詳細填單回傳,如班上均無符合者,亦請務必於三週時限內(9月26日前)完成回傳,未回傳者視同無特定人員提列。
三、另學期間若發現學生施用或持有不明藥物,或觀察生理或生活狀態,認有可疑為施用或持有毒品之情形,亦請致電校內分機2202、2203洽生輔組承辦人關長順先生協處。

1.In accordance with the Ministry of Education’s official letter No. 1142803363 dated July 24, 2025, and Article 4, Section 1 of the university’s Guidelines for Urine Testing and Counseling of Designated Individuals, each semester within three weeks from the start of classes, homeroom teachers, course instructors, professional counselors, or staff members are required to observe and report students who fall under the categories of designated individuals. The list shall be submitted to the Office of Student Affairs for compilation and reviewed in a designated meeting.
2.Homeroom teachers (including department chairs) are requested to, within three weeks from the start of the first semester of Academic Year 114 [no later than Friday, September 26, 2025, before the end of office hours], observe whether any students in their classes or surrounding environment show signs of being “reasonably suspected of using drugs.” If such cases exist, please complete the online form via the provided link or QR code (see below). If no students meet the criteria, please still submit the form before the three-week deadline (by September 26). Failure to submit will be deemed as no designated individuals being reported.
3.In addition, during the semester, if any student is found to be using or possessing suspicious substances, or if observations of their physical condition or lifestyle suggest possible drug use or possession, please immediately contact the Student Assistance Section at extension 2202 or 2203 (Mr. Guan, Chang-Shun) for follow-up assistance.
